To Claim Sales Tax Exemption

Please note: All orders sent to the following states will be charged sales tax initially, but once we have the proper documents we will refund the sales tax.

To obtain a refund of your sales tax, attach scans of the required documents to the form below and submit, or fax them to us at 206-352-9492.

If Paying with Credit Card, required documents include:

1. A copy of a state resale certificate, exemption certificate, or other acceptable proof of your exempt status for the state where the items will be shipped; and
2. Proof that your payment was made with your organization's credit card by submitting a photocopy of the credit card. For security purposes, black out the first twelve digits.

If Paying with Check, required documents include:

1. Payment can also be made with a corporate check. In that case we only need a copy of a state resale certificate, exemption certificate, or other acceptable proof. No additional photocopy of the check itself is needed.
